Consent for Emergency Medical Treatment and Medication
Any child in attendance who becomes ill, has a contagious disease or condition, or suffers an injury that requires medical attention shall be separated promptly from the group, but shall have continuous supervision by a school staff member.
The ill or injured child's parent/guardians shall be notified and required to pick up, or arrange for another designated person to pick up the child. Emergency contact information and information regarding who is allowed to pick up each of your children MUST be kept up-to-date in the school's files.
If your child requires medication from a physician, please fill our a Medication Authorization Form located in our Director's office.
In the event immediate and/or emergency medical treatment should become necessary and required, and the parent/guardians or other designated person cannot be reached prior to such emergency medical treatment being determined to be required and necessary, St. James Episcopal School will obtain and authorize such necessary and required medical treatment as recommended by a medical professional. However, all medical expenses of any kind shall be the sole responsibility for the party or parties signing the financial responsibility for the applicant on the enrollment form.
Please provide a copy of your child's insurance card to keep on file.
